Students must officially register in order to take classes at the Salt Lake Valley Institute. This helps us to keep track of students' progress and provide the best services possible in an emergency situation. Registration is also necessary for a student to receive credit for Institute.
The Student Registration form is required for ALL students. The additional Institute Scheduling Request form is only necessary if a student attends an affiliated post-high program (such as Canyons Transition Academy, Granite Post-High School, or the Salt Lake School District SCORE Program. )

- Click on the yellow "Submit" button at the top of each form. This will automatically open your computer's email client so you can send us the form OR
- Save the filled-out form as a PDF and email it to [email protected] OR
- Print out the filled-out form and give to any member of our faculty. That works, too.
